In this training, you’ll learn what care coordination means and why it’s important.

Sometimes, a client may get help from more than one home care agency (HCA) or provider. Your job is to tell your supervisor if you know of any other people or agencies helping the same client. This helps your team stay organized and provide the best care possible.

Care coordination means making sure:

• There are no overlaps (two people doing the same task),

• And no gaps (no one doing an important task).

Your supervisor is in charge of making sure all services are working together smoothly. But you play an important part by sharing what you see and speaking up about any concerns.

When everyone communicates and works together, clients stay safer and get better care.
